Enrollment & Forms

 2017-2018 SCHOOL YEAR

Enrollment packets can be accepted at the FCCC Main Office or turned to our FCCC staff during program hours.

To complete the enrollment process FCCC will need the following items for each child:

ENROLLMENT PACKET

FCCC is a state licensed program. All information and forms in the enrollment packet are required, including the Virginia CACFP Meal Benefit Income Eligibility Form (the parent letter explains how to complete that form). 

MANDATORY RECORDS

  • If not already on file: the child’s original birth certificate (for verification purposes only)
  • If not already on file: a physical or well check, no older than 2 years, signed or stamped by physician (NEW ENROLLEES)
  • If not already on file: immunization records, signed or stamped by physician (NEW ENROLLEES).  The following School Entrance Health Form can used for your child's health records; Part I is completed by a parent or guardian, Part II and Part III must be completed by the Medical Provider.
  • Health Record Release Form If you have turned in your health records to your school they can make a copy for us upon completion of this form.  Please submit to your school's main office.

TUITION

  • A $55.00 non-refundable registration fee (per child) and fees for the first month enrolled are due at the time of sign-up. 
  • Payment Methods: Check, Cash or Money Order only. Checks must be written in black or blue ink.
  • August Tuition Chart  Fees are 1/2 price for August
  • September-May Tuition Chart  Fees are calculated for the year and broken down into 9 equal payments. The full monthly amount is due each month regardless of the number of program days. Fees are not reduced or refunded for non-school days or extended absences (i.e. Winter Break, Spring Break, inclement weather, etc.) Additional fees are assessed for Holdiay and Full Day Teacher Work Day Care Days.

CHILD CARE ASSISTANCE
Qualifying
for a sliding scale tuition rate (Step 1 or Step 2 fees) requires proof of your annual household income and completion of our Income Verification Form. Your annual income includes all wages, child support, social security, etc. You will be charge full fee tuition until required documentation is received and verified.  Two most recent pay stubs are required for proof of income.  If you have been approved for assistance through the DSS Daycare Assistance Program FCCC will need to see your verification letter from DSS.  To contact the Department of Social Services for assistance call (540) 422-8450.

CHANGE OF CARE
If you need to change your care package (type of care) the following form must be completed: Change of Care Payment Agreement.
Once complete the form can be turned in to your FCCC site staff or our Main Office staff, e-mailed, or faxed (540) 347-3518.  We request 24 hours notice to change your care. 

 PAYMENT BOOK
You will receive a payment book at the time of enrollment.

WITHDRAWING FROM FCCC
To avoid being charged tuition beyond the last month you plan to attend a notice of withdrawal should be made in writing by the middle of the month.  Withdrawal must be received by the administrative office a minimum of two weeks prior to the last date of the child's attendance.

  •  Fees are due for the entire enrollment period. 
  • Tuition is a monthly charge and is not prorated for withdrawal. 
  • Once complete the following form can be taken to the FCCC center your child attends, e-mailed or faxed to the Main Office at (540) 347-3518. For additional information please refer to the School Year Handbook.

MEDICATION CONSENT AND SPECIAL DIET FORMS

If your child requires emergency medications the following forms must be on file along with the prescribed medication:

If your child's enrollment paperwork indicates they have a medical need and you choose not to have medication on site with FCCC the following form must be on file:

 

If you child has food allergies and will need an alternate snack the USDA Special Diet Form needs to be completed in its entirety for FCCC to remain in compliance with USDA regulations.
